Brain Injury Background:Traumatic brain injury (TBI) is an insult or trauma to the brain caused by external mechanical forces. Patients with TBI are often left with significant cognitive and behavioral disability.
There are no effective standard treatments for TBI patients.
At the time of injury there are different degrees of irreversible damage to the brain tissue termed the primary injury. Next, there is ongoing injury to the brain including microglial activation, excitotoxic neurotransmitters and impaired calcium balance. mTBI & PCSUp to 90% of TBIs are classified as mild TBI (mTBI). Concussion is a form of mTBI and most symptoms resolve in 5-10 days. Post Concussion Syndrome (PCS) is a set of symptoms that remain beyond 14 days. The PCS symptoms include headache, dizziness, neuropsychiatric (mood) symptoms, and cognitive impairments. In most patients, PCS may continue for weeks or months, and up to 25% of the patients may experience Prolonged PCS (PPCS) in which the symptoms last for over six months. Hyperbaric OxygenAt any given time, the brain is utilizing almost all oxygen and glucose delivered to it. Concussion & Brain Injury - Human DataAt our clinic in Edmonton we offer some of the most modern alternative treatments to address neurological injury including Hyperbaric Oxygen & IV Therapy. The following section will discuss human data regarding the use of Hyperbaric treatment for brain injury and concussion starting with case reports before transitioning to clinical trials. Case ReportsThere is preliminary case report data for use of Hyperbaric Oxygen for Brain Injury - the results are extremely compelling. Harch (2009) - A 25-year-old male military veteran with Post Concussion Syndrome and PTSD three years after an explosion. 39 Hyperbaric Treatments were done at a pressure of 1.5 ATA. The patient experienced a permanent marked improvement in his post-concussive symptoms, physical exam findings, and brain blood flow as well as a complete resolution of PTSD symptoms. Similar to this case report, we frequently use 1.5 ATA pressures and blocks of around 40 treatments for treating brain injury at our Edmonton clinic. Wright (2009) - 2 US servicemen Post Concussion Syndrome with symptoms including irritability, sleep disturbances, headaches, memory and cognitive difficulties. 40 - 80 Hyperbaric Treatments were were done at a pressure of 1.5 ATA. Both patients experienced rapid improvement of headaches and sleep disturbances, and improvement in all symptoms and eventual resolution of most symptoms. Once again, this case report demonstrates efficacy of a moderate pressure such as 1.5 ATA in Brain Injury. The need for a total of 80 treatments in the second case speaks to the idea that some personalization is needed. At our Edmonton clinic, a break of 4 weeks is reasonable between treatment blocks to allow the brain to rest from the consistent oxidative load. Shytle 2019 - 3 veterans with chronic Traumatic Brain Injury and PTSD symptoms. 20-35, Hyperbaric Treatments were were done at a pressure of 1.5 ATA (60 minutes). Case 1: Improvements in judgment, memory, social interactions, anxiety, and headaches. Case 2: Reduced anxiety, better sleep and motivation, memory and mood. Case 3: Improvements in impulsivity, anxiety, fatigue, sleep and depression. Harch (2012) - 16 veterans with mild to moderate Traumatic Brain Injury with Post Concussion Syndrome and PTSD. 40 Hyperbaric Treatments were were done at a pressure of 1.5 ATA (60 minutes). 12 of 15 (80%) had an improvement in the majority of their symptoms with some having a reduction in their PTSD symptoms. The patient population included higher severity brain injury (moderate TBI) and still yielded overwhelmingly positive results. This is likely a population where some of the patients would have benefitted from a second block of treatment which we would have endorsed for our Edmonton brain injury and concussion patients. Regular IV Therapy treatments (1x weekly) has proven sufficient for numerous PCS/TBI patients with a goal of cessation of treatment within 12 treatments. Our Naturopathic Doctors usually suggest 40 treatments with most Brain Injury and Concussion and then add blocks of 20 as needed. Do I need a consultation before treatment?

The changes in SPECT images after treatment indicate that Hyperbaric Treatment led to reactivation of neuronal activity.
