Do I need a consult before receiving iron? 

Of course you do.

But don't worry, your initial iron treatment consult is free.


This complimentary visit takes about 15 minutes, and we can usually find you a spot within about a week.


Who supervises your iron therapy? 

All iron therapy at our office is supervised by our nurse practitioner.

How long does it take to before I can get treatment?

We can usually get you in for a treatment within a week or two.

This is booked after you have completed your complementary iron consult.


What does iron treatment cost? 

Treatment fee is $265 - you'll have a dedicated nurse supervised by our Nurse Practitioner.

Costs of iron vary depending on doses used - often times between $300 to $600.

Many patients are able to receive reimbursement through their benefits for the cost of the iron.


How does iron therapy work? 

Iron therapy, bypasses the digestive tract and provide clinically relevant doses.


Iron therapy has unique physiology for absorption and distribution.


An iron-sugar complex is taken up by the immune system and the free iron that is released is stored as part of the ferritin (protein) or taken to various cells by transferrin. 


How often does someone need iron treatment? 

This depends on initial ferritin and hemoglobin.

You will be sent for testing approximately 4 weeks later. 


Patients often require one or two treatments within about four weeks of each other.


How effective is iron therapy? 

This also depends on initial ferritin and hemoglobin.

Often times, a single treatment can raise ferritin levels between 80 to 150 (µg/L) depending on the dose chosen as well.


How safe is iron therapy? 

In about 5% of patients mild adverse effects can be seen within the first three to four days after finishing iron therapy.


The incidence of anaphylaxis is about occurs with a very low probability, with estimates of severe reactions like anaphylaxis ranging from approximately 1 in 250,000 to 1 in 10,000 administrations, rather than 0.004%

All About Iron

Iron Absorption


The body absorbs different forms of dietary iron at varying rates.


Non-heme iron, which comes from plant-based foods and most oral supplements, is absorbed less effectively—typically only 2–20%.


Heme iron, found in animal sources such as meat, poultry, and fish, is absorbed most efficiently, with rates ranging from 15–35%.


Absorption of non-heme iron can be reduced by natural compounds in plant foods, such as phytates (found in grains and legumes) and polyphenols (present in tea, coffee, and certain vegetables).


Vitamin C significantly enhances non-heme iron absorption, making it an important dietary factor for individuals relying on plant-based iron sources.


Stomach acid also plays a key role by converting ferric iron (Fe³⁺) into the more readily absorbed ferrous form (Fe²⁺).


At our Edmonton clinic, we use clinically effective and highly bioavailable forms of iron therapy.


Measuring Iron Levels

No single laboratory test provides a complete or definitive assessment of iron status.


Instead, a panel of complementary tests offers the most accurate evaluation.


Serum ferritin remains the primary marker of iron stores and is the most commonly used indicator in clinical practice.


However, ferritin can also behave as an acute-phase reactant and may be elevated in the presence of inflammation or infection. 


Serum iron, transferrin saturation index, and total iron-binding capacity (TIBC) provide insight into circulating iron and transport capacity.


A complete blood count (CBC)—with close attention to hemoglobin concentration and mean corpuscular volume (MCV)—helps assess the impact of iron deficiency on red blood cell production.


At TruMed in Edmonton, our practitioners incorporate all of these measures when evaluating patients, though we primarily rely on ferritin levels as the central marker of iron status, while interpreting results in the broader clinical and biochemical context.


Iron Biochemistry

Iron is best known as a key component of hemoglobin, making it essential for carrying oxygen.

Iron is also integrated into the electron transport chain, where it supports mitochondrial energy production.

It is also necessary for the conversion of tyrosine into dopamine involved in mood and cognition.

 Iron Deficiency Therapy

Most patients begin treatment with over-the-counter (OTC) iron supplements, as they are widely available, cost-effective, and often sufficient for correcting mild deficiencies.


However, clinical studies report that up to 70% of patients experience gastrointestinal side effects—including nausea, constipation, diarrhea, or abdominal discomfort—from conventional iron preparations.


In addition, standard formulations may be ineffective in normalizing ferritin or hemoglobin in a significant subset of patients, particularly those with malabsorption, inflammatory conditions, or intolerance to higher doses.


At our Edmonton clinic, we frequently encounter cases where iron therapy has not adequately corrected deficiency.

Iron Infusions in Edmonton

At our Edmonton clinic, we provide medically supervised intravenous (IV) iron infusions for patients with documented iron deficiency or iron deficiency anemia. While some patients refer to these treatments as “iron transfusions,” the correct term is iron infusion.


Unlike oral supplements, IV iron bypasses the digestive tract entirely, allowing for rapid and reliable restoration of iron stores.


A single iron infusion can result in a rise of ferritin levels by 100–200 µg/L, depending on baseline deficiency and dosage administered.


This makes IV iron one of the most effective strategies for correcting moderate to severe iron deficiency, particularly in cases where oral iron is poorly tolerated or ineffective.

How Iron Infusions Work

IV iron is formulated as an iron–carbohydrate complex, consisting of an iron hydroxide core stabilized by carbohydrate ligands. This structure prevents immediate release of free iron, allowing for controlled delivery and minimizing oxidative stress. Once infused, the complex is taken up by macrophages, where iron is gradually released.


It is then either stored in ferritin or transported by transferrin to the bone marrow and other tissues for red blood cell production and metabolic needs.


In Canada, two primary formulations are available:

  • Monoferric® (ferric derisomaltose) – Preferred in most cases due to its ability to deliver up to 1,000 mg in a single infusion, reducing the number of sessions required.

  • Venofer® (iron sucrose) – More commonly used in pregnancy, as it is given in smaller divided doses.

Both products differ in molecular weight, carbohydrate composition, and infusion protocols, but are highly effective for restoring iron levels. At our Edmonton clinic, Monoferric® is the most frequently administered preparation, while Venofer® is used in specific clinical contexts, such as pregnancy.

Safety and Tolerability of Iron Infusions

When administered under proper medical supervision, iron infusions are very safe. At our Edmonton office, all treatments are performed by trained nursing staff under the direction of our Nurse Practitioner.

  • Mild infusion-related reactions (seen in ~5% of patients): flushing, headache, muscle or joint discomfort, flu-like symptoms, abdominal upset, or temporary rash.

  • Delayed reactions (less common): hives, joint swelling, fever, diarrhea, or muscle cramping.

  • Severe reactions: The risk of anaphylaxis is extremely rare, estimated at 0.002–0.004% (1 in 25,000 infusions) depending on the iron formulation used.

Infusions typically take 45–60 minutes, with careful monitoring throughout the procedure. Patients are advised to avoid dislodging the IV, as extravasation of iron can cause permanent skin staining.

Iron Dosing at Our Edmonton Clinic

Dosing is determined using the Ganzoni formula, which takes into account baseline ferritin, hemoglobin, and body weight. Typical doses range from 500–1,000 mg per infusion, depending on clinical need.

  • Most patients require only 1–2 infusions to restore ferritin and hemoglobin to target ranges.

  • Follow-up blood work is arranged 4–6 weeks post-infusion to assess response and determine if further treatment is required.

  • Special considerations: We do not administer IV iron in patients with thalassemia unless iron deficiency is confirmed, nor in males under age 18 without documented low ferritin.

Pregnant women can safely receive Venofer®, typically administered in multiple smaller infusions as per obstetric guidelines.

The cost of the iron product itself typically ranges from $300–600, depending on the type and dose required.
